Medicaid - What If This System Is Necessary

Limits on Medicaid Payments

The standard governing when and how much Medicaid must pay is that it must pay for all services which are "medically necessary." Many states create definitions of this term in an effort to control or contain costs. Many examples of such limitations exist and are being employed in the private insurance sector today, as well. Pre-approved services and coverages may be necessary. Certain limitations above specified statutory numbers, e.g., number of therapy visits, may apply and to effectuate coverage, prescription by a Medicaid-approved physician may be necessary.
